Pilot action
Telemedicine Hub for Specialist Access in the Town of Cres Based on the EDiT Model
The core concept of this pilot is the establishment of the Cres health centre as a digital gateway between patients and a regional hospital on the mainland (Rijeka). The pilot adapts this ‘telemedicine hub’ approach from the EDiT telemedicine model developed in the South Aegean (Greece) in a small-scale and feasible test aligned with Interreg Europe requirements.
The system addresses the issues of access to medical services for islanders by providing remote access to specialties that rely mainly on data analysis and visual/verbal interaction rather than physical examination:
• endocrinology (data-driven chronic disease management)
• psychiatry and psychology (video-based consultations)
• neurology (cognitive monitoring and follow-up)
The local GP in Cres plays a central coordinating role. A limited number of patients (up to 10) will also use simplified digital tools (tablets and medical kits) for logging health data.
The pilot will test:
• operational workflows between island primary care and mainland specialists
• reliability and effectiveness of digital communication and data exchange
• usability for healthcare professionals and patients
• clinical usability of remote consultations
• reduction of unnecessary patient travel
• integration in existing healthcare structures
Patients will participate in scheduled tele-consultations supported by real-time or recorded health data (e.g. blood pressure, glucose, oxygen saturation).
The technical setup includes:
• 1 hub at the Cres health centre
• 1 specialist hub at the regional health centre in Rijeka
• 2 physician tablets for local GPs
• up to 10 patient tablets with integrated monitoring solutions + measuring medical equipment kits
The pilot is expected to produce concrete results in terms of validating the transferability of the EDIT telemedicine hub solution to Cres island context; tackling operational, technical, and organisational challenges; and providing evidence-based recommendations for policy improvement.
